A Pastor’s Recommendation is required to apply for enrollment at Spiritual Life Bible College. A link with simple instructions is included upon the completion of this application.
If you have not attended or completed college: You must submit a photocopy of your high school transcript, diploma, or GED certificate along with this application as indicated by the question above.
For Bible Colleges, other Colleges or Universities attended: You must have original, sealed, official transcripts sent directly to us from your college/university to the following address:

This form does not put you into “contractual obligation” to pursue a degree or diploma with Spiritual Life Bible College, nor hold the student to graduating within a certain timeframe. The form will be placed in your file and will help the administration to streamline both our offerings and your College experience. Signing this intent to pursue a degree or diploma program helps SLBC know what classes to offer which quarters, so we can graduate you more quickly. Our advisors can help you find what classes are best to take, when to start your practicums, ministry formation programs, senior thesis, other requirements. Finally, this form cues us in to see how we can best help you with transfer credits from other educational organizations, on-line courses, etc.
Student Classification:Degree candidates are classified on the basis of quarter credits earned.
Freshman: Student has less than 45 quarter hours of college credit.Sophomore: Student has a minimum of 45 quarter credits.Junior: Student has a minimum of 90 quarter credits.Senior: Student has a minimum of 135 credits.
Master of Bible Theology: Requires 70 graduate level creditsB.A. Biblical Theological Studies: Requires 192 creditsA.A. Biblical Theological Studies: Requires 96 creditsBible Diploma Certificate: Requires 48 credits.
A full-time load is considered to be 12 quarter credits per quarter.
Students may take individual classes without declaring a degree program, however a student application must be submitted before taking classes.
Students may apply for a ministerial license after earning 48 credits of Bible either in the diploma program or degree programs. However, they must meet the ministerial guidelines of Covenant Life Ministerial Organization and obtain interviews with the organization. Contact Covenant Life Ministerial Organization for more information on the requirements.

Privacy Rights of Students: STATUTE 20, UNITED STATES CODE, 1232g and regulations adopted pursuant thereto. The code provides for an institution to establish a category of student information termed “directory information.” All information, such as health and medical records, disciplinary records of personal counseling, required student and family financial income information records, transcripts or student permanent academic records, student placement records and other personally identifiable information shall be open for inspection only to the student and such members of the professional staff of the college as have responsibility for working with the student. Except as required for use by the president in the discharge of his official responsibilities as prescribed by laws, regulations of the state board, and board policies, the designated custodian of such records may release information from these records to others only upon authorization in writing from the student or upon a subpoena by a court of competent jurisdiction.

I certify by my signature that I agree to abide by the policies of this institution as described in the SLBC Student Handbook and Course Catalog. I understand that SLBC is accredited by ACI, a private nongovernmental agency and is not regionally accredited. ACI has not sought U.S. Department of Education recognition at this time nor does ACI guarantee the acceptance of academic credit by colleges and universities or other Bible schools. I acknowledge that SLBC is not a job placement service and make no claims regarding employment.

All students admitted to the Bible College are expected to refrain from the use of any tobacco, alcoholic beverages, illegal drugs and abuse of habit-forming prescription or non prescription drugs, both on and off campus site. No improper language (cursing) will be tolerated. Students will also maintain behavior above reproach in relationships with the opposite sex, avoiding all appearances of evil. Sexually immoral living (non-marital, homosexual, or lesbian) is not acceptable. Anyone found to be engaged in these activities may be dismissed from the College. In accordance with the Drug-Free Schools and Communities Act Public Law 101-226 and in adherence to God’s principles, Spiritual Life Bible College is a drug and alcohol free college. No abuse of this law will be tolerated.

Students should be appropriately dressed to represent the ministry, the college, and SLBC-affiliated events. Students will not be allowed to use extreme dress or fashion. SLBC staff reserves the right to determine the appropriateness of all dress. In general, students are expected to dress in “business causal” wardrobe. Clothes should be neat and clean. Women can wear dresses, skirts with sweaters or blouses. Dress pants or pants suits are also appropriate. No low cut necklines or dresses with splits above the knee are permitted. Men should be well-groomed, neat and clean. Men can wear casual slacks and shirts with collars. Earrings are not acceptable for men. Jeans, shorts, and t-shirts are considered inappropriate and should not be worn.
